Intermediate Care is a collaborative service between Health and Social
Services. It is a time limited
service (1-6 weeks) and aims to prevent long stay in hospital.
It involves active treatment and rehabilitation and hopes to keep people
in their own homes (preventing premature or inappropriate admission to hospital)
