Community Care

 

What are Community Care Services?

 

Community Care Services are services arranged by Social Services.  The types of services which may be available are:

 

·        Advice and support with personal or family problems

·        Home care to provide personal care and essential domestic care

·        Help with meals

·        Specialist equipment

·        Home adaptations such as a ramp, grab rail, etc

·        Moving to specially-adapted or sheltered housing

·        Attending a group day centre or training centre

·        Care in a residential or nursing home for either day care, respite care or long-term care

 

If you think may need any of these services, you need to request an assessment from      Social Services.

 

Assessments

An assessment is carried out by an assessing social worker who makes a formal visit to look at whether someone needs to receive either Health Service care or Social Services community care.  Anybody who thinks they need help in their daily living is entitled to request an assessment.  See Social Services for more information about assessments.
